Yes, you can login to our Member Portal – myhealthmembers.com to change your address or you can use the Change Request Form included in this brochure. It is important to tell us that you have moved because we provide your address to the claims payer and your claims may not be paid.

It depends on the provider, although you can file a claim yourself by submitting a claim form to the address on the back of your Insured Medical ID card. Always check with your provider to ensure you are aware who is responsible for filing the claim.
A claim form must be completed within 90 days after the covered loss begins or as soon as it is reasonably possible. Ask your provider to file the claim and send it to the address on the back of your ID card.
Champion Benefit Advisors does not pay claims. To check the status of a claim or if you receive an Explanation of Benefits (EOB) from the claims payer stating that no benefit is being paid, please contact the insurance carrier directly.
